Immunization Requirement

 

Your student account will incur a REGISTRATION HOLD if complete immunization records are not received by our office.

 

How Do I Know My Hold Was Lifted?

  • Visit https://my.gwu.edu/login/.
  • Login to the GWeb Info System.
  • Select the following: Student Records & Registration, Student Records Information Menu, View Administrative Holds.
  • “Incomplete Immunization Status” means your record was either not received or is incomplete. If there is no Student Health hold listed, your record is received and complete.

 

 

Who Must Comply

ALL students under age 26,

regardless of program of study, part-time or full-time, degree or non-degree status must comply with the immunization requirement as set forth by the Law of the District of Columbia.

 

Students age 26 or over,

are automatically exempt from university immunization requirements. Medical and Health Science students should contact their academic department for additional requirements. 

Deadline to Submit Forms

 

All students must have forms submitted by:

Aug. 1st  for Fall admits

Jan. 1st  for Spring admits

 

 

 

 

What is Required

 

Mail or fax proof of the following immunizations:

1)      one tetanus shot in the past 10 years

2)      two MMR (measles, mumps, rubella) vaccinations after age 1 and at least one month apart

3)      Documented history of chicken pox

        -OR- two chicken pox vaccinations

4)      Medical Students: all of the above + Hepatitis B vaccinations (3) + Hepatitis B titer

5)      International Students: #1-3 (above) + TB screening (PPD) strongly recommended

 

 

 

 

How to Submit Forms

 

The following documents are sufficient in place of a clinician's signature:

1) Childhood immunization records

2) Clinician records

3) High school or college records

 

*** Print and fill-out the Student Health Service Immunization Form.

*** Medical, Physician Assistant (PA), and Sonography students must submit BLUE CARDS with immunization information.

 

 

*** Make a copy of the SHS form and your official documents for your records. *** Documents may be sent to Student Health directly from their source, e.g. your clinician's office or your past school.

Medical or Religious Exemptions

Students seeking exemption from the immunization requirement for medical or religious reasons must submit the following:

1) a letter from a medical clinician or church clergy stating the reason for a need for exemption

-AND-

2) titer (blood test) results for Measles, Mumps, Rubella, and Varicella (chicken pox)

 

 

 

 

Processing Time

4 weeks by mail

3 business days if forms are faxed or walked into our office

 ** We do not process records on Saturdays & Sundays

 ** During class registration periods, records are processed in 24 hours or less.
