Why do I have a HOLD on my registration?
If you have not submitted the mandatory immunization form, and have no knowledge of this requirement, please visit http://gwired.gwu.edu/shs/ImmunizationRequirement/ for more information. If you are sure that immunization forms have been sent to Student Health on your behalf, it is possible that your records were incomplete. If this is the case, call (202) 994-6827, and press option 2.
I submitted immunization forms and have a registration HOLD.
If you are sure that the mandatory immunization form was sent to Student Health on your behalf, and there is still a HOLD on your account, it is possible that your records were incomplete. To inquire about incomplete records, call (202) 994-6827 and press option 2.
Why was I able to take classes last semester if my forms weren't processed?
Your first semester at the university is a grace period for submitting the immunization form. Registration holds are placed on student accounts when immunization forms are not submitted by the registration period for 2nd semester classes (usually late October, early November for Fall admits).

What are the requirements for Medical and Health Science Students?
Medical and Health Science Students under age 26 must mail or fax proof of the following immunizations to Student Health:
One Polio vaccine (All Students under 18)
One Tetanus vaccine in the past 10 years
Two MMR (measles, mumps, rubella) vaccinations after age 1 and at least one month apart. -OR- Lab Report showing positive immunity
Three Hepatitis B vaccines -OR- Lab Report showing positive immunity
Two Varicella vaccines (Chicken Pox) -OR- Documented history of chicken pox -OR- Lab Report showing positive immunity
One Meningococcal vaccine -OR- Meningitis Wavier Medical Students: all of the above + Hepatitis B titer (blood test which proves immunity)
International Students: All of the Above + TB screening (PPD) strongly recommended

What are the requirements for International Students?
International students under age 26 must mail or fax proof of the following immunizations to Student Health:

When should the mandatory immunization form be submitted?
The mandatory immunization form and/or supporting documentation should be submitted by August 1st for Fall enrollment or January 1st for Spring enrollment. If your forms are not submitted by the registration period for your second semester at the university, your student account will incur a registration HOLD.
Note: Your first semester at the university starts when you start taking classes, regardless of your student status (part-time, full-time, non-degree, etc.).
I am unable to get a clinician's signature.
You do not need a clinician’s signature and stamp if you provide one of the following: childhood immunization records, school records (e.g. high school, undergraduate university), clinician records.
Where can I find a copy of my records?
Check with your family to see if they have a copy of your immunization records available. A copy of your records may also be on file with your previous place of education (high school, undergraduate university). Records may also be found at your doctor’s office. Please be aware that these records may not be up to date.
I am compliant with my home state’s immunization policy; do I still need to submit immunization records?
Yes, all students must submit immunization records. Immunization requirements differ by state.
I don’t have any immunization records.
If you are sure that you have been immunized against the measles, mumps, and rubella, and/or the chicken pox, you have the option of getting your titers drawn. A titer is a blood test which tests your immunity to certain diseases. Positive blood titer results illustrate immunity, and are sufficient documentation for the immunization requirement. There is no titer available to determine whether you have been vaccinated against Tetanus and Diphtheria; if you have no documentation of this vaccination, you will have to be re-immunized.
