PAE & Health Form
This double-sided form comprises a Participant Agreement & Emergency Contact Information form as well as a Health form. It is required for all TRAiLS outings. You'll need your health insurance information to complete it, and if you're under 18 years of age, your parents will need to sign it as well. You will generally complete this form when you register for a TRAiLS trip - this is usually done in the office of the Student Activities Center (Marvin Center suite #427). Once you have completed this form, it gets filed in the TRAiLS office for the entire academic year.
Trip Evaluation Form
This form helps us improve. It also helps us know which outings you'd like to see planned in the future. So please take the time to give us your feedback! If you didn't get a chance to fill out an eval form after your trip, please take this opportunity to complete one now! Evaluation forms should returned to our office (Marvin Center #436). Thanks!
