At the student's request, DSS prepares individualized letters to professors which verifiy the nature of the student's disability and notes the need for accommodations. Students must make a new request for letters each semester and are responsible for delivering these letters to their professors, who are not obligated to meet a request for accommodations without prior notification of the student's needs.

You may obtain a Letters to Professors Request Form from the DSS office or download it from our website. The completed form should be submitted to the DSS office for processing.
