Guidelines for Documentation of Temporary Conditions

Students may present with a temporary condition, such as physical injury, which may impact their academic their academic performance. Access to accommodations through Disability Support Services may be deemed appropriate to support those needs.

If you require assistance with participation in the academic program due to such circumstances you must furnish to DSS a statement on letterhead completed by a qualified clinician which provides:

  • a description of the problem
  • an assessment of how the injury compromises the student's academic performance 
  • a prognosis or expected length of impact on the student 
  • suggestions for needed accommodations or services based on the list of services found in our services guide

Please see the Services Guide for a list of services or accommodations that the University can provide a student with a documented disability or temporary condition. Services not offered by DSS may often be found by contracting with a local provider.  Please see our Community Resources page for more information. Upon registering a request for this assistance through DSS, students will be given information about how to access those accommodations identified for their use.
