Establishing Your File

STEP 1: Get Registration Form
You must establish a file before you can start using DSS services. Click HERE to print out a copy of registration form. Should you need any assistance in completing this form, please feel free to stop by.

STEP 2: Submit Information
Students must complete and return the form along with documentation of disability to DSS. For documentation guidelines, click here.
Please note that an unsigned or incomplete registration form will NOT be processed. Registration information sent via e-mail is invalid and will not be processed.
STEP 3: Process Period
Once the registration form and documentation are received, DSS will start to process your request. Given the volume of requests, it may take two weeks to complete the review process. You will receive written notification of your eligibility for disability support services. We look forward to assisting you.
