Preparing for Initial Appointment

The first appointment at UCC is a telephone appointment with a counselor. The receptionist will schedule a specific time, the same day or usually within 24 hours, for the intake clinician to call you at the telephone number that you provide to us. It is important to be available for approximately 20-30 minutes and to be able to speak privately at the time of the call. During this call the counselor will speak with you about why you are contacting us and will also ask a series of questions about different areas of your life in order to accurately assess your treatment needs. At the end of the phone call, your counselor will offer you his or her professional recommendation on what type of treatment services, if any, would be most appropriate for you. Here are some examples of what might happen after the initial appointment:

If short-term individual counseling at the UCC is recommended, you will be assigned to a UCC staff counselor based on compatible schedules and special needs that you may have. You will then be set up for a UCC appointment or be referred to the UCC case manager to arrange for an appointment to be made.

We ask that you fill out the initial appointment paperwork, which includes a Welcome Letter/Informed Consent, Demographic Information, and Confidential Personal History, and bring it with you to your initial counseling appointment. If you can, it would be helpful to review these forms before your telephone assessment with the counselor. When you arrive for your counseling appointment, give your completed forms to the receptionist, who will give them to your counselor to review before your session. You can download these forms from this site by clicking on the documents below:

If you and your initial appointment counselor determine that our time-limited counseling services are not appropriate for you, your counselor will discuss alternative counseling options that you can pursue in the community. Our UCC case manager is available to assist all clients with getting connected to outside referrals, whether through insurance plans or sliding scale fee-based providers.
