Immunization Form

The District of Columbia Immunization Law requires that all students under 26 years of age provide written documentation of immunity to Measles, Mumps, Rubella, Tetanus-Diphtheria, and Varicella (chickenpox). The completed Immunization Form must be returned to the Student Health Service by mail before August 1, 2006.  Alternately, you may bring the completed form with you to CI this summer.  You may not be allowed to attend classes if the completed form is not on file with the Student Health Service by the end of the first week of classes.  The University does not require physicals as a pre-matriculation requirement.

Student Consent for Release of Information/Certification of Financial Dependency

The Family Educational Rights and Privacy Act (FERPA) of 1974, as amended, seeks to guarantee both a student's right of access to records and the confidentiality of student information. A student's record may be released to parents only if one of the following conditions have been met: 

  1.  through the written consent of the student, 
  2.  in compliance with a subpoena,
  3. in connection with some health or safety issue, or
  4. by submission of evidence that the parents declare the student as a dependent on their most recent Federal Income Tax form, as defined by the Internal Revenue Code of 1986, Section 152.

The Release Form grants a student's parent/guardian to have access to his/her academic record. Either the student can give his/her consent for the parent/guardian to have access to their educational record OR the parent/guardian can certify that the student is a dependent as stated on the most recent federal income tax return for the tax year 2005. This form must be submitted each calendar year directly to the University Registrar, at the address below.  For more information on the consent form, click here.
